CPS Transportation Request Form

This bus request form is reserved for school personnel to complete for school-sponsored extracurricular activities or field trips.

Before submitting your transportation request, please note the following:

Please submit your request at least 3 weeks before your scheduled event. If you have an alternate date, please list that.
Capacity- The CPS bus has 52 seatbelts. However, that assumes every seat on the right side has 3 occupants and every seat on the left has 2 occupants. The older and closer to adult size the kids are, the lower the space and capacity.
Each CPS van can hold a maximum of 8 people, INCLUDING the driver.
All bus and van requests will be processed in the order in which they are received.
The CPS bus can only be used for trips that are 60 miles one way or less.
Monday-Thursday during CPS programming season, the bus must be returned to St. Anthony's by 2pm to ensure that the driver can re-fuel if needed and do the afternoon run.
