I hereby consent to be interviewed or photographed by: the Catholic Partnership Schools organization, the five schools in the Partnership (Holy Name, St. Anthony of Padua, St.Cecilia, St. Joseph Pro-Cathedral, and Sacred Heart), public media (eg: the Catholic Star Herald) and/or the Diocese of Camden, and their trustees, directors, members, officers, employees, volunteers or agents (hereafter known as “CPS”).

I hereby grant to CPS the right to edit, reproduce, use, and re-use images for any and all purposes, including but not limited to, advertising, promotion and display, and I hereby consent to the editing, reproduction, use, and re-use of said images in any and all media in existence and all media yet in existence including, but not limited to, video, print, internet, television, radio and podcasts.

I forever grant, assign, and transfer to CPS any right, title, and interest that I might have in any images taken of me at CPS or engaged in a CPS-sponsored activity. I hereby agree to release, indemnify, and hold harmless CPS from any and all claims, demands, actions or causes of actions, loss, liability, damage or cost arising from this authorizing.

I understand that I may revoke this agreement at any time in writing to CPS, and that the revocation will only apply to future use of my images.

I have carefully read and fully understand the authorization, release, and agreement outlined above prior to signing it.
